Help explain please

So I’m pretty sure dr Sarno says as long as nothing is structurally wrong it’s TMS. Also he believes that herniated disc’s do not cause pain. So is a herniated disc not considered a structural problem? which one is it?
 
It's not a problem in the sense that a herniated disc does not correlate with pain or other symptoms. It's simply a "normal abnormality " as Dr. Sarno put it. Herniated discs are incidental findings (not problems) and the medical world has historically linked causality of herniations to pain. There are numerous studies debunking this as a spurious correlation. "Correlation is not causality'.
