The best advice I can give you to say that yes, sciatica and piriformis are absolutely TMS. After truly believing that you have TMS and knowing other people have recovered from what you are experiencing just stop thinking about anything related any other name (piriformis) for your TMS...

Diagnoses received: sciatica, piriformis syndrome, gluten intolerance, anxiety, PTSD
MRI's or tests taken: Normal Lower Back X-ray, Normal gallbladder ultrasound.

I personally think psychosomatic pain is a mighty fine descriptor but people are very defensive. I like TMS too, Sarno was clearly trying to find the perfect way to describe the pain and the source of the pain and I have so much respect for that.

I found that I was not able to get out of pain if I continued to entertain any hint of "physical" causes for my pain. Even a sliver of distraction into the physical cause/cure would fuel the pain.
I avoided gluten for many year because abdominal pain. It "worked." But when I had success with another type of chronic pain using the info here I started to think the gluten intolerance might be TMS. Yep! I now have a much less restricted diet and have started baking bread as a hobby!
Steps? I don't think that just because many people find writing helpful that writing per se has to be an essential "step." I'm not a strong writer, it is a struggle for me. Therefore journaling isn't an important healing activity for me. BUT I consider myself a TMS success. Keep exploring this...
