I was just at the doc's for a minimally invasive, relatively new procedure called Cooled Radiofrequency (RF), and was given the usual clipboard at reception to fill in the mandatory boilerplate check boxes. I just recommended in a post for the umpteenth time for someone to explore the Rahe-Holmes list of stressful life situations that can cause TMS/psychosomatic dis-ease:

When modern medicine advances to accept stress as a cause for illness, the Rahe-Holmes list should be part of the required boilerplate information to fill out before seeing the doc.
