That is the case for me, too. There is no correlation between being physically tired and being sleepy for me. My fight/flight mechanism gets stuck in the "on" position. I suspect it is due to my high ACE score. I am hypervigilant and unable to feel safe enough to let myself go into sleep. Occasionally I can fall asleep without medication and sleep 7-9 hours without waking up, but lately never more than 2 nights in a row. Most difficult TMS equivalent for me to get a handle on.

Ellen, yes, this was exactly my case - you described it precisely. And it is absolutely related to the emotional disturbance: anxiety, fear, depression. Interesting that my ACE score is not high, but I had circumstances outside my family that were dramatic, painful and dominated most of my childhood and my adult life into late 30-ies.

I am seeing improvement with meditation. The more I meditate, the better I sleep, the less symptoms I experience. Last night, my close family member was taken to the hospital with a stroke - my symptoms are going through the roof today, my sleep was disturbed, depression is rising. I am trying to meditate, but find it very difficult.
