Pilonidal sinus and anal fistulas

Depends on what the medical pros say. If they can't explain why you have this, OR why these symptoms happen, OR offer you a well-established treatment that will solve the problem... you might as well go the TMS route because it is 100% non-invasive, therefore you have nothing to lose by doing so.

Edit: my answer may seem a bit dismissive, but this isn't your first time with new symptoms, so I'm assuming that you know the drill.

they have no idea and blame “ingrown hairs”, its bullshit and makes no sense and the surgeries are 50 percent chance success. After asking a few mind body coaches and thinking about it on a deeper level i believe it is tms!
