The main mission of our brain is twofold: survival and reproduction. That mission is tasked to the primitive or reptilian brain and ensures the survival of our specie.

In order to seek oportunities for reproduction we "fall in love"
In order to avoid dangers (real or supposed) menacing our survival we ...
"fall in pain"

Far from our will (the Autonomic Nervous System rules!) our reptilian brain floods the circulatory system with hormones: dopamine, serotonin, oxytocin,...(to fall in love) and cortisol,... (to fall in pain)
To get its goal the brain confuses us. We fall in love thinking that we love someone, that she or he is our half orange, that passionate romance is true love. We fall in pain thinking that our body is damaged or in danger. That could be true... or not. A snake could be dangerous... but a "suspicious branch" not.
The reptilian brain is preventing us from thinking: flight or fight (or freeze). Emotions are not material things but could be much more threatening, or the protecting brain thinks so: pain (more or les persistent) and/or a myriad os weird symtoms that will get to catch our attention, to create worries and preocupation, even fear or panic, and to focus in our body. So, a perfect smoke curtain is created, the emotional turmoil is bypassed and the remote, but feasible, possibility of thinking about suicide ... braked.

Falling in love ... and falling in pain .... so far, so near.
