Look, I've dealt with perfectionism for as long as I can remember.

I'm sitting here, looking at my messy room (which isn't really messy but my parents like to hassle me for it being messy) and thinking... maybe it's TMS. Maybe dealing with I have to clean my room changes the focus of whatever I am seriously frightened of.

Could this be the same reason why I have a potential client for business who wants to pay me money, and I have been delaying getting back to them?

Personally, I think it is probably just part of your personality/defense mechanism thing.
Cleaning your room probably feeds more into the child/parent relationship which for you, still sounds like there is somewhat of child still revolving around that because you are living at home and (I presume, because you have clients) you are an adult. It's probably much less about fear and much more about your emotions towards your parent and the role you felt you fill.
The most important thing is how you feel about these things - how does being an adult and having to live up to your parents standards make you FEEL (don't answer, just think about it). How does your need to be "messy" (or untidy or whatever they judge you as) fulfill in your life if anything at all. I imagine this is all repeat patterns.
How do you feel about meeting the business client? How do you feel about your own procrastination?
TMS is the mind/body connection - your personality and psychology is part of this, but lots of people who do not have pain or physical symptoms also engage in these types of patterns and behaviors.
